Job Description
We are looking for Care Managers/Social workers are employed in the Older Peoples Assessment and Care Management Team. They are accountable via their Team and Senior Manager to the Service Manager and Head of Service and ultimately to the Director of Social Services. They are expected to undertake Assessments, develop care packages and pursue care management work in accordance with the Council's function under the Social Services Act 1970 as set out in Schedule I of that Act, Community Care Act 1990 and other relevant legislation.
