Job Description
A position has become available for a qualified social worker to join the Older Persons Swift
Team based at Mary Seacole House in Clapham, this is a full time post, 35 hours per
week.
Candidates must have post qualified experience, preferably within a hospital setting dealing with
safe and timely discharge.
A good working knowledge of recent legislations is needed along with Fair Access to Care, Delayed Discharges Act, SAP and FACE.
