Job Description
A highly skilled Social Worker is required for our multi-disciplinary team in West London, providing an effective and efficient assessment and care management service to vulnerable adults.
To carry a caseload and work with service users, carers and other professionals to undertake timely assessments, produce care plans, care manage, review and monitor service users, within the requirements of Council policy, with specific reference to the NHS and Community Care Act and other relevant legislation and guidelines.
